- Bali Visa application form.
- Colored copy of the identity and signature pages in your passport.
- Passport valid for at least 18 months with available blank pages.
- Copy of KITAS (temporary stay permit).
- Valid domicile letter in Bali.
- Birth certificate.
- Blood type.
- Recent passport-style photo with white background.
- Local sponsor.
- Indonesian national ID card (KTP).
- Regional Taxpayer Identification Number (NPWPD).
