To obtain this certificate, make sure to provide the following:
Bali Visa application form.
Colored copy of the identity and signature pages in your passport.
Passport valid for at least 18 months with available blank pages.
Copy of KITAS (temporary stay permit).
Valid domicile letter in Bali.
Birth certificate.
Blood type.
Recent passport-style photo with white background.
Local sponsor.
Indonesian national ID card (KTP).
Regional Taxpayer Identification Number (NPWPD).
