This document is issued to foreigners in Indonesia and serves as official proof of residence. It can also be used for address mutation requests or KITAS extension applications.

To obtain this certificate, make sure to provide the following:
- Bali Visa application form.
- Colored copy of the identity and signature pages in your passport.
- Passport valid for at least 18 months with available blank pages.
- Copy of KITAS (temporary stay permit).
- Valid domicile letter in Bali.
- Birth certificate.
- Blood type.
- Recent passport-style photo with white background.
- Local sponsor.
- Indonesian national ID card (KTP).
- Regional Taxpayer Identification Number (NPWPD).
